We are really fortunate to have a terrific working relationship with the staff and physician members of Northwest Physicians Network (NPN). We started working together even as the idea of the Referral Service was a vague — albeit shared — sketch in the minds of NPN’s CSIO Rick MacCornack and Clarity’s CEO Peter Gelpi.
The service is based on the premise that coordinated patient care is better patient care. With a community-based referral management service, Primary Care Physicians in that community can work with all their consultants using the same simple tools, and with them provide a coordinate care experience that ultimately leads to better health outcomes. PCP’s staffs leave the hassles of insurance checking to Clarity, removing a cumbersome, time-consuming and error-prone task from their busy agendas. It frees up clinic staff to more closely interact with patients, schedule follow-ups, and manage care registries.
These tools are enablers in NPN’s strategy to provide the means for their PCP members to be modern, effective, efficient and sustainable “medical homes” for their patients.
